STOCKHOLM (Reuters) ? Novartis's bone drug Zometa extended survival in older breast cancer patients but failed to improve disease-free survival among younger women patients in a large-scale clinical trial, researchers said on Sunday.
Detailed data presented at the European Multidisciplinary Cancer Congress (EMCC) in Stockholm showed that Zometa, a bisphosphonate drug known generically as zoledronic acid, only improved overall survival rates in patients who had undergone the menopause at least five years earlier.
Researchers said the effect was likely to be linked to levels of reproductive hormones, which are lower in women who have been through the menopause.
"The results have shown that zoledronic acid should not be used routinely for the treatment of early breast cancer. However, for those with low levels of female hormones, due either to natural aging or specific treatments to induce menopause, the approach appears very promising, as more women go on to rid themselves of the disease," said Robert Coleman of Britain's Sheffield University, who led the study.
He said the strong results in older women could lead to "a major new treatment approach" for post-menopausal patients.
Zometa, which generated sales of $2.1 billion for Novartis in 2010, is an intravenous drug from a widely used class of osteoporosis medicines called bisphosphonates.
It is increasingly being prescribed to help reduce or delay fractures and other skeletal complications in a variety of cancers that have spread to the bones. From 65 to 75 percent of breast cancer recurrences occur in the bones.
This trial, called AZURE and also published in the New England Journal of Medicine on Sunday, involved 3,360 women with early-stage breast cancer from 174 centers in Britain, Australia, Spain, Ireland, Portugal, Taiwan and Thailand.
Patients randomly allocated receive standard chemotherapy and/or hormone therapy, either with or without the addition of Zometa, a drug that works by inhibiting the cells that break down bone, called osteoclasts.
The results showed that looking across all trial participants, there was no significant difference in disease-free survival or overall survival between patients who received Zometa and those who didn't. But in a subset of older patients, the addition of the Novartis drug did bring clear benefits.
These women, who had at least five years since their last period, had an overall survival rate at five years of 85 percent on Zometa compared with 79 percent for those not on the drug.
"This is a small but significant increase," Coleman told reporters at the Stockholm conference.
For all other patients, overall five-year survival was similar, at 86 percent in the Zometa group and 85 percent in the control group.
Investigators said that most surprising among the findings was the drug's effect on the rate at which the cancer recurred outside the bone. The relative risk for developing metastases, or secondary tumors, outside bone during treatment with Zometa was approximately halved in the postmenopausal women compared with the younger patients.
Coleman said the results shed new light on the role bones may play in the progress of the disease.
"The effects on metastasis and recurrence outside bone suggests that the bone marrow is an important sanctuary for tumor cells which can be activated after, sometimes, many years of dormancy," he said. "With help from bone marrow stem cells, they may then spread via the blood stream to set up metastases at other sites."

Audiences will find no humor in The Tempest. What they will find is an abridged story of an overtly wronged wronged woman accused of murder by her brother-in-law. She loses everything and is set off in a raft with her young daughter to die. She doesn?t though. Twelve years pass then she gets a chance to protect her daughter and even things.
Building the script on the gimmick of casting the main character Prospero as a woman instead of a man is appropriate to the 21st Century not the 1600?s. Focusing on Prospera, her adult daughter and a shipwrecked crew full of enemies, the plot is easier to follow than it as a traditional Globe Theater romance. The expected pastoral scenes (rugged, lovely, volcanic Hawaii location), pre-Christian elements (Ariel and Caliban?s characters), magic and fantasy (Prospera?s illusion and manipulation), and a happy ending (Miranda and Ferdinand together) are there.
Critically speaking, the desolate, remote, isolated, unspoiled natural location is the perfect choice for setting The Tempest. However, setting too many visual against it is somewhat distracting. Elements of the soundtrack made it difficult to interpret some lines. The sheer power of words rather than special effects makes the story. Makeup and costuming for this period piece brings the right feel, tone, and movement to the characters. The casting of Hillen Mirren as Prospera is perfect; Alfred Molina?s Stephano has true comic flair.

With Apple poised to release its newest iPhone model, an unsurprising poll has found iPhone users are least likely to switch to devices made by another manufacturer.
A survey by market research firm UBS of 515 smartphone users around the world found that among iPhone users, 89 percent have no plan to switch, far higher than the retention rate for HTC, at 39 percent; Research In Motion's BlackBerry devices, at 33 percent (down from 62 percent in March 2010); Samsung, at 28 percent; and Nokia, at 24 percent (down from 42 percent in March 2010).
Worldwide Popularity
iPhone users made up the largest share of the sample, at 45 percent, far more than the competition, with the next largest share belonging to Canada-based RIM with 18 percent, followed by Finland-based Nokia with 16 percent.
The largest share of respondents were European, 40 percent, while 36 percent were in Asia and 23 percent in the Americas. About 1 percent of respondents resided in Africa.
Of those using Android-based phones, 55 percent said they would stick with the platform while 31 percent planned to get their hands on an iPhone. Taiwan-based HTC, maker of the Droid Incredible, Thunderbolt and other smartphones, was the most popular OEM among those using Android-based devices, with a 39 percent share, compared with 27 percent for Korea-based Samsung and 18 percent for U.S.-based Motorola.
"In general, retention rates appear to be falling for most of the OEMs," said UBS in its report. "Relatively, Apple's retention rates have held up incredibly well even as its market share has risen. Interestingly, when we look at all consumers who are considering changing handset OEM provider, Apple remains a significant net beneficiary. Alongside Apple, only Samsung and HTC also appear as net beneficiaries (more users won than users lost)."
The report noted that among the 10 percent of current Apple users not committed to sticking with iPhones, just 6 percent indicated they were certain to switch, while 4 percent suggested they were still in play, which means a potential retention rate for Apple as high as 93 percent, depending on whether the iPhone 5 wins them over.
Above Average Popularity
The iPhone consistently polls high in customer satisfaction. Earlier this month it won an 838-point rating in a survey by J.D. Power and Associates, far above the industry average of 788.
But with so many rivals emulating the touch-screen interface, app-driven smartphone experience, is Apple really so far ahead of the competition in quality, or just better at marketing?
"Apple is a marketing machine," said Alex Spektor or Strategy Analytics. "They did a tremendous job on the PC side, on the MP3 side and now on the phone and tablet side. They know how to sell and their users know how to sell their product for them.

President Obama allegedly sought Warren Buffett's permission to use his name for the so-called Buffett tax -- the proposed rule that would ensure those making more than $1 million a year don't end up with lower tax rates than middle-income workers. That was smart. The tax might be one of the most controversial proposals of Obama's presidency.
Some say it's a jobs killer. Others say it's class warfare. A comparison to socialism inevitably pops up. The responses are as predictable as they are impassioned.
In the spirit of adding fuel, here are four points about the Buffett tax to keep in mind.
1. Buffett has his own take on class warfare. "Class warfare may make for really good politics, but it makes for rotten economics," said Congressmen Paul Ryan, referring to the Buffett tax. He's right. Any policy that takes aim at one class is bad economics, if only because it creates the perception that those elected to serve you are out to get you.
But Buffett, the Berkshire Hathaway (NYSE: BRK-B - News) billionaire, has his own take on class warfare: "There's class warfare, all right, but it's my class, the rich class, that's making war, and we're winning," he's said in the past.
There are no specifics of how the Buffett tax might work, but initial descriptions outline that its goal is to ensure that the highest earners pay at least the same tax rate as middle-income workers. This isn't about taking one group of taxpayers who already pay abnormally high tax rates and pushing those rates higher. It's about flattening the tax code so that some with the highest incomes don't end up with some of the lowest tax rates, as is now the case.

See the craziness? $999,999 of income faces a $150,000 tax bill, while $1 million faces $200,000 in taxes. If that's the case, people would rationally jump through hoops to lower their income.
To be fair, this is how it appears on the surface, but no one knows how the Buffett tax might work in practice. There are no details yet.

Finance Minister Evangelos Venizelos rushed to allay fears the canceled trip signaled imminent default, saying such talk was "ridiculous," but the conservative opposition seized the opportunity to demand snap elections, fanning fears Greece lacks the will needed for tough measures ahead.
"The comments and analyses about an imminent default or bankruptcy are not only irresponsible but also ridiculous," Venizelos said in a statement.
"Every weekend Greece ... is subject to this organised attack by speculators in international markets."
Papandreou was in London, en-route to United Nations and International Monetary Fund (IMF) meetings, when he decided to turn back after discussing developments with Venizelos, government officials said.
"The prime minister judged that he should not be away. He wants to ensure that all of Greece's commitments (to its EU partners) are fulfilled," government spokesman Ilias Mossialos told Reuters.
A government official speaking on condition of anonymity told Reuters pressure was high on Athens from euro zone partners to take additional measures to merit continued funding from a 110 billion euro ($150 billion) bailout to avert default.
"There is an issue of trust. Our partners want very specific steps and commitments and our record so far unfortunately does not inspire confidence," said the official.
Next week, Greece is due to resume talks with EU and IMF inspectors who will judge fiscal progress before releasing the next 8 billion euro loan tranche in October.
Greece has said it has cash until next month.
"It's a sign that things are very tight. Papandreou's presence is crucial to make sure there are no setbacks with issues that need to be resolved," said Theodore Krintas, head of wealth management at Attica Bank.
ELECTION CALL
The conservative opposition New Democracy party, which voted against the bailout that saved Greece from bankruptcy last year, seized the opportunity to make a fresh call for snap elections.
"The only solution is elections, so that the people's will is expressed," New Democracy leader Antonis Samaras said in a speech in the northern city of Thessaloniki.
New Democracy, which is leading the ruling socialists in opinion polls, says the policy mix used cannot bring Greece out of the crisis and austerity measures were stifling the economy.
The conservatives are riding a wave of public discontent after two years of austerity measures and are proposing tax cuts and growth boosting measures instead.
"When a policy hurts my country, I will surely say no. Why should I co-sign a mistake?" Samaras said. "We want this destructive policy toppled."
Apart from the slow pace of reforms and fiscal slippages, international lenders are most concerned with the lack of political consensus in Greece. Even if elections are held in 2013 as planned, the next government must apply agreed policies for the country to recover.
The ruling socialists have a majority in parliament but political analysts say internal dissent and public unrest, such as strikes and violent protests, may prompt snap elections.
Fiscal slippage this year, which the government blamed on a deeper-than-projected recession, forced Athens to slap a levy on property to make up for the shortfall as a target of capping its budget deficit at 7.6 percent of gross domestic product looked out of reach.
Lenders have long warned against one-off measures and more taxes as a way out of the crisis shaking the euro. They have asked for urgent reforms and privatisations and a drastic shrinking of the bloated public sector.
EU economic and monetary affairs commissioner Olli Rehn has said inspectors from the European Central Bank, EU and IMF would report back on progress in early October, meaning the next disbursement of aid to Greece could be paid by mid-October.
A second 109 billion euro bailout agreed in July, after it became clear Greece would not be able to return to bond markets, has also hit snags.
Euro zone partners are asking for collateral before giving Athens more cash and banks are slow to participate in a bond swap scheme key to the deal.
Papandreou was to meet United Nations Secretary-General Ban Ki-Moon in New York on Sunday and IMF head Christine Lagarde on Tuesday. Venizelos is still due to attend an IMF meeting in Washington later in the week.

The number of young children admitted to hospitals or seen in emergency departments because they unintentionally took a potentially toxic dose of medication has risen dramatically in recent years, according to a new Cincinnati Children's Hospital Medical Center study.
The rise in exposure to prescription products has been so striking that the Centers for Disease Control and Prevention has established the PROTECT Initiative, intended to prevent unintended medication overdoses in children.
Randall Bond, MD, an emergency medicine physician at Cincinnati Children's, will present his study on children and pharmaceutical poisonings Sept. 20 at a PROTECT Initiative meeting in Atlanta. The study will be published online Sept. 16 in the Journal of Pediatrics.
"The problem of pediatric medication poisoning is getting worse, not better," says Dr. Bond, who also is medical director of the Drug and Poison Information Center at Cincinnati Children's. "More children are exposed, more are seen in emergency departments, more are admitted to hospitals, and more are harmed each year."
Dr. Bond found that exposure to prescription products accounted for most of the emergency visits (55 percent), admissions (76 percent) and significant harm (71 percent). Levels of ingestion of opioids, most often prescribed to treat pain; sedatives-hypnotics, frequently prescribed as sleep aids; and cardiovascular medications were particularly high.
"Prevention efforts at home have been insufficient," says Dr. Bond. "We need to improve storage devices and child-resistant closures and perhaps require mechanical barriers, such as blister packs. Our efforts can't ignore society's problem with opioid and sedative abuse or misuse."
Dr. Bond studied patient records from 2001 to 2008 in the National Poison Data system an electronic database of all calls to members of the American Association of Poison Control Centers. Dr. Bond studied children 5 years old and younger exposed to a potentially toxic dose of a single pharmaceutical agent, either prescription or over-the-counter. A total of 453,559 children were included in the study.
The largest part of increasing admissions, injuries and death was due to children finding and ingesting medication on their own. Therapeutic errors at home were uncommon and increased only minimally.
The most likely explanation for these trends is a rise in the number of medications around small children, he says. A 1998-99 survey found that half of adults had taken at least one prescription medication in the preceding week and 7 percent had taken five or more. In 2006, the same surveyors found that 55 percent had taken at least one prescription medication in the preceding week and 11 percent had taken five or more.

SOUTH CHARLESTON, W.Va. ? -(Ammoland.com)- The next West Virginia Archery in the Schools (AIS) Program instructor training session will be Nov. 29 ? 30 at the Gander Mountain Store in South Charleston, according to Krista Snodgrass, AIS program coordinator for the West Virginia Division of Natural Resources. The registration deadline is Nov. 11.
The AIS Program is the DNR?s largest and fastest growing educational program. Since its inception in 2004, approximately 60,000 students have been introduced to archery as part of their physical education (PE) classes in 248 West Virginia schools. The program teaches Olympic-style target archery in both public and private schools? Grade 4-12 PE classes.
?In order for schools to have the AIS Program, they must first have a certified instructor ? preferably a PE instructor or coach who will introduce as many kids as possible to the joys of target archery,? said Snodgrass. ?The DNR offers about four trainings a year; and, because class size is limited to 25, training slots are highly sought after. To ensure a training slot, we encourage PE teachers and coaches to apply as soon as possible.?
As an added incentive, teachers taking this training may be eligible to receive Continuing Education Hours.
This training will cost $25 for all school employees and $50 for non-school employees. This fee will be paid to ?WV DNR ? Archery in the Schools Program? by check, money order, or exact currency during the first morning of the training.
